Required Clearances and Background Checks

Employees are required to have the following clearances/forms:

  • Criminal Background Check - Act 34

  • PA Child Abuse History Clearance - Act 151

  • Federal FBI Criminal History Records - Act 114

  • Sexual Misconduct/Abuse Disclosure Release - Act 168

 All clearances/forms are to be turned in to the Business Office.  Clearances are good for 5 years.

PSP Criminal Background Check: Act 34

A criminal background check must be completed on every employee!

There are two options to request a criminal record. All applicants are strongly encouraged to use the PATCH system.

  1. Register on-line at PATCH (Pennsylvania Access to Criminal History): EPATCH

    • Please Note: You will need to use a credit card for the on-line registration

  2. Click on submit a new record check and follow instructions

  3. When you get your search results, click on the CONTROL # and then click on CERTIFICATION FORM

  4. Print the "Record Check Details"

The phone number for the State Police regarding Background Check reports is: 1-888-QUERYPA (1-888-783-7972).

When you receive your background check, bring it into the Business Office.

PA Child Abuse History Clearance: Act 151

A child abuse history clearance must be completed on every employee

There are two options to request a PA Child Abuse Clearance:

  1. On-line Access: https://www.compass.state.pa.us/CWIS

    • Please Note: You will need an email address to create an account

  2. Click on "Create Individual Account" and follow the instructions

    • Submitting your clearance online will give you immediate access to your results or the status of your results if your results cannot be processed immediately.

You can access the website again by clicking on "individual login" to go back into retrieve your results.  It is your responsibility to provide the Business Office with a copy of the results.

Federal FBI Criminal History Records: Act 114

FINGERPRINT-BASED BACKGROUND CHECK PROCEDURE - Act 114

Effective April 1, 2007, Act 114 of 2006, Section 111 of the Public School Code was amended to require all school district employees to provide to their employer a copy of their Federal Criminal History Record

Current Act 114 Clearance (Federal Fingerprinting) IdentoGO :

  • Schedule or Manage Appointment - Complete Form

Sexual Misconduct / Abuse Disclosure Release: Act 168

Under Act 168 of 2014, effective December 22, 2014, all applicants must complete a Sexual Misconduct/Abuse Disclosure Release Form for your current employer(s), and for all former employers if the former employer was a school entity and/or where the applicant had direct contact with children. Once Greenwood receives these completed forms back from you, we will mail them to your current and former employers. Employers will have 20 days to return the completed forms to us. After we have received all the completed forms back and they are satisfactory, then we can consider you for employment. Forms are available in the district office & online.
